I have the same problem and I think I know what wrong with your CM4. You have purchased CM4 with MMC and you should flash it: using SD card looks like won’t work.

My IO board will arrive on friday and I will report here if this solves the issue.
Cheers @Zimbatron, mine definitely isn’t an emmc model though as it doesn’t have an emmc chip on the board.

I know sound funny have you tried turning raspberry pi around soon as I did this after 4 hrs troubleshooting it working for me haha
OK so it looks like the SDRAM on my CM4 had failed.
I put it in the IO board, and had the green led flashing on and off 8 times which is the error code for faulty SDRAM.
Popped the new CM4 into the IO board and it booted right up.
Tried it in the uConsole (still in its deconstructed state) and I have output on the HDMI and then the screen!!!
Will put it all back together later today and test everything works.

I really hope so! I got it from Mouser so will contact them and see what they say
